Required Documents at Registration:
You must bring with you a COPY of the participants State Certified Birth Certificate (not issued from hospital), a 2x3 photograph, and a Current Sports Physical dated after January 1st, 2012. The Participant must be present at registration for uniform fitting and initial weigh-in. We will also require (2) copies of the completed Spring 2012 report card prior to equipment pick-up. No equipment will be issued until all documentation has been provided as players are not eligible to participate until certified. No exceptions will be allowed!
Registration Fees:
The registration fee for the 2012 Tackle Football and Cheerleading season is $160.00. Registrations start March 1, 2012 at Hazel Dell Elementry School from 6:30 to 8:30. Tackle Football participants will keep their game jersey with their last name on the back and mouth guard. The registration fee includes the rental of helmet, shoulder pads and game pants.
The registration fee for the 2012 Cheerleading participants will keep the Uniform. The registration fee includes the Uniform and SWWA competion fee that takes place at the end of the season. Practice package information will be available at registrations.
Discounts:
We have limited scholarships for our 2012 season. All applications need to be turned in by our April registration date with all required documentation. this includes Household funds of anyone that is 18 and older living in the house. We need a copy of you 2011 W-2 and your last 2 pay stubs. Any goverment provided documentation for child support, social securties ect...
Refunds:

No verbal requests will be granted. For Flag: All written refund request recieved on or before April 9, 2012 will be granted a full refund minus a $10.00 administrative/processing fee. Any refund request after April 9th will be granted a refund minus a $10.00 processing fee and at $25.00 jersey fee. No refund request will be granted after April 15, 2009 For Tackle/Cheer:All written refund requests received on or before July 31st, 2012 will be granted a full refund minus a $25.00 administrative/processing fee. Any subsequent written refund requests received on or before August 7th will be granted a 50% refund (minus a $25.00 administrative/processing fee). Requests made after August 1st are also subject to an additional deduction of $50.00 if a jersey has already been ordered. The player will be allowed to keep the jersey if ordered. No refund requests will be granted after August 7th.
